Position Summary

We are currently seeking a Social Program Officer to join our team in the Boundary.

The Social Program Officer provides comprehensive assessment, care planning and treatment coordination for primary health care patients and their families. Working collaboratively with physicians and Primary Health Care Nurses in the patient medical home (doctor or nurse practitioner’s office) as well as with other Community and Integrated Health Services providers, the Social Program Officer provides direct psychosocial care, counselling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating patient self-management. 

In addition to providing psychosocial supports to patients, the Social Program Officer will play an integral role in the advancement of the patient medical home/primary care home model through participating in continuous quality improvement processes, training in teamwork and communications, regular clinic team meetings, quarterly regional meetings, and supporting integration of existing health authority services into the patient medical home.

The Social Program Officer will be employed by the Boundary Health Care Cooperative, a community services cooperative created to facilitate the provision of integrated clinical health and wellness services in community-based primary care settings within the Boundary Region. The Social Program Officer will provide care to patients in patient medical homes (doctor or nurse practitioner’s office) in the communities of Grand Forks, Christina Lake, Greenwood, Midway and Rock Creek.

Qualifications:

  • Bachelor’s degree in an Allied Health, Behavioural or Social Science.
  • Two years recent related experience in a health setting working with complex care patients with concurrent disorders.
  • Current valid BC Driver’s License.
